Level 2, 4, and 7 paratracheal lymph nodes are generally accessible to the mediastinoscope (Figure 1). Relative contraindications include prior thoracic radiation, previous mediastinoscopy, aortic arch aneurysm, tracheal resection, or functioning tracheostomy.
